Job description

Job title: Practice Nurse
Location: Balderton Primary Care Medical Practice
Salary: Up to £48,000 Depending on Experience
Hours: Part-time (27.5 hours)

Key Responsibilities:
As a valued member of the team, your key duties will include:

  • Assisting the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained.
  • Working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
  • Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
  • Providing a wide range of nursing services to patients, including chronic disease management, health screenings, and immunizations.
  • Conducting health assessments and screenings, including blood pressure checks, vaccinations, and general health checks.
  • Supporting patients with long-term conditions (e.g., diabetes, asthma, COPD) and assist in the creation of care plans.
  • Administering vaccinations, travel health advice, and general health education.
  • Performing routine treatments, such as dressings, injections, and minor wound care.
  • Supporting with the management of health promotion and disease prevention within the practice.
  • Taking patient histories, carrying out clinical assessments, and offering advice on lifestyle changes and disease prevention.
