Job summary

APEX PCN serves a population of nearly 47,000 patients. The PCN has been selected to take part in an NHS England Pilot to improve patient access. The aim of the hub is to absorb same day patient demand enabling practices to focus on population health and chronic disease management. We are seeking a new enthusiastic team member to join this fixed term exciting opportunity.

We welcome applications from both nursing and paramedic backgroundswith a prescribing qualification. Primary care experience is beneficial.

Main duties of the job

We are seeking a motivated Practitioner to work in our newly setup 'First Contact' hub. You will be seeing patients who have been triaged from across our 4 PCN practices at a centralised location. These patients will have one issue requiring same day assessment and management. Complex patients e.g. pregnant patients or those on palliative care register would not be eligible to be seen in the acute hub.

The hub will initially operate between 9am and 5.30pm with the majority of patients being seen in face to face consultations, the candidate may be asked to amend hours once the hub develops. The hub patient list and consultation notes will be recorded on System One. The successful candidate will have the opportunity to help us shape this exciting new service alongside the APEX PCN team.

About us

APEX PCN is located in West Lincoln and incorporates 4 practices Birchwood Medical Practice, Boultham Park Medical Centre, Richmond Medical Centre and Woodlands Medical Practice. We have a population of 46,000 with a predominantly younger demographic. The PCN is supported by a strong ARRS team. This includes pharmacists- engaged in supporting practices in their daily workload while also focussing on the DES requirements. We have an effective FCP team supporting practices for MSK care and a well-developed EHCH and Frailty MDT to support care homes. Our practices work collaboratively to achieve our IIF targets and PCN DES requirements. We are now looking at forming strategic partnerships with our other Health and Care colleagues to effectively address the health needs of our population.

Job description Job responsibilities

The post holder is an experienced Nurse/clinical Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the PCN. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

The Candidate will:

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

Complete the relevant training in order to provide multi-professional clinical practice.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication within the urgent care service and with GP practices and external stakeholders.

Act as an advocate for patients and colleagues.

Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

The post holder will maintain the security of sensitive personal and other confidential information and will work within all relevant Information Governance policies reliably to working practice. Additionally, they will be expected to follow secure operating procedures for handling information in all formats, including verbal, written and that held electronically.

The post holder will have the appropriate level of child protection and adult safeguarding knowledge, skills and practice required for the post; be aware of and comply with child protection and adult safeguarding policies and procedures.

This post will be subject to a DBS (Disclosure and Barring Service) Check

In addition to observing the host Practices rules and procedures, which all staff are required to observe and follow, the post holder is also required to follow general policies and procedures that apply to the employment relationship.
