You will support high‑quality patient care through clinical reviews, preventative health work and treatment room activity, with clear documentation in the clinical system. Immunisations, wound care and recall management are part of the role; long‑term bookings

Qualifications

• NMC-registered Nurse with experience in primary care.
• Up-to-date immunisation and cytology training desirable.
• Confident in chronic disease management and treatment room-work.
• Good patient communication and clinical reasoning.
• Familiarity with EMIS, SystmOne or Vision.

Responsibilities

• Conducting chronic disease reviews for patients with asthma, COPD, diabetes or hypertension
• Performing wound care, dressings and treatment room duties
• Carrying out adult and childrens' immunisations (if trained)
• Undertaking smear tests (cytology-trained nurses)
• Completing NHS Health Checks and lifestyle assessments
• Managing results, recalls and routine follow-up tasks
• Documenting clearly in the clinical system
