Locum Ready is seeking a Locum Practice Nurse to work with a GP practice in Hereford. The role focuses on chronic disease reviews, wound care, and routine nursing clinics, in a structured primary care setting.

You will contribute to high-quality patient care through clinical reviews, preventative health work and treatment room activity, with well-organised workflows and supportive nursing leadership. Flexible locum and long-term bookings are available.

Qualifications

• NMC-registered Nurse with experience in primary care.
• Up-to-date immunisation and cytology training desirable.
• Confident in chronic disease management and treatment room-work.
• Good patient communication and clinical reasoning.
• Familiarity with EMIS, SystmOne or Vision.
• Enhanced DBS

Responsibilities

• Conduct chronic disease reviews for patients with asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carrying out adult and children’s immunisations (if trained)
• Undertaking smear tests (cytology-trained nurses)
• Completing NHS Health Checks and lifestyle assessments
• Managing results, recalls and routine follow-up tasks
• Documenting clearly in the clinical system

This role is ideal for a Practice Nurse with strong assessment skills who can confidently deliver chronic disease reviews, treatment room procedures and routine nursing clinics in a structured primary care environment.

You will contribute to high-quality patient care through a balance of clinical reviews, preventative health work and treatment room activity. The practice offers well-organised workflows, supportive nursing leadership and a clear structure for daily clinic delivery.
