Due to the expansion of the nursing team, The StationPractice is looking for a practice nurse.

We are proud to have an extended clinical team of Doctors, Nurses, Mental Health nurses, FirstContact Physiotherapists, Pharmacist Practitioners and Health Care Assistants.You will also be supported by an efficient, friendly administration team.

Main duties of the job

The Nursing Team delivers clinical, evidence-based practicefor our local population, meeting the needs of patients with particularattention to those with long-term conditions and the provision of preventativehealthcare. The ideal candidate should have experience working in areas ofChronic Disease Management, Cervical Cytology, and vaccinations/ immunisations.In addition to this, the candidate should have experience in providing routinehealthcare assistance, wound care, well-woman and well-man clinics, and supportingthe team with clinical emergencies.

Job responsibilities

The Practice Nurse works independently, taking initiativeand being accountable for their professional actions. They administer immunisationsand vaccinations for adults and children according to national and localguidelines. The nurse is competent in anaphylaxis and resuscitation techniques,ensuring the safe storage, rotation, and disposal of vaccines and medicationswhile maintaining adequate stock levels.

The Practice Nurse provides comprehensive travel healthadvice to patients prior to travel, covering topics such as malariaprophylaxis, safe sex practices, food hygiene, sun protection, first aid,emergency medications, health insurance, and vaccinations. They perform variousprocedures, including venepuncture, glucose tolerance tests, blood pressuremanagement, urinalysis, peak flow measurements, spirometry, new patient healthchecks, pregnancy testing, stitch or clip removal, ear syringing, cytology,wound management, and cryosurgery. Additionally, they offer a comprehensivefamily planning service.

The nurse understands the importance of observing,documenting, and communicating the signs and symptoms of child and adult abuse.They work in accordance with relevant practice and Clinical Commissioning Group(CCG) policies and procedural guidelines, including infection control,chaperoning, and patient group directions.

Key Role: Care and Programme Management

The nurse conducts chronic disease monitoring clinics withinthe practice to meet the requirements of the Quality and Outcomes Framework(QoF). They initiate the diagnostic process for patients suspected of havingchronic diseases such as diabetes, Chronic Obstructive Pulmonary Disease(COPD), asthma, or Coronary Heart Disease (CHD), referring them to otherclinicians as appropriate. The nurse maintains disease registers, audits careoutcomes against established standards, and makes necessary changes to caredelivery. They conduct annual reviews to assess patients' understanding andability to self-manage their conditions and refer them to other members of theextended primary healthcare team, such as chiropodists, dietitians, andspecialist nurses.

Key Role: Health Promotion

The Practice Nurse initiates and carries out healthscreening programs, enabling, encouraging, and supporting individuals,families, and groups to address issues that affect their health and socialwell-being. They represent the interests of individuals and families when theyare unable to advocate for themselves. The nurse provides advice on healthylifestyles and smoking cessation to individuals and groups and develops andmaintains health promotion materials within the practice. They also makereferrals to statutory and voluntary agencies as appropriate.

Key Role: Manage Self

The nurse contributes to the development, monitoring, andevaluation of care delivery by the nursing team. They promote effectivecommunication and collaboration within the team to achieve shared objectives.The nurse participates in clinical supervision for their own development, recognisesthe developmental needs of others, and provides clinical supervision toHealthcare Assistants (HCAs). They actively engage in clinical and practicemeetings and work to enhance their knowledge and practice in response toobjectives and changes in services.
