Job summary

Ambrose Avenue Group Practice invites an experienced practice nurse to join our busy Colchester team on a full-time or part-time basis. The role covers health assessments, vaccinations, and management of long-term conditions across two sites.

You will run asthma, COPD and diabetes clinics, perform phlebotomy and ECGs, and contribute to patient-centered care and health promotion. NMC-registered nurses with GP experience are preferred.

Qualifications

  • Must hold a recognised nursing degree and be NMC registered with an active PIN.
  • Experience in general practice nursing with competence in chronic disease clinics.
  • Desirable modules in GP nursing or equivalent experience.

Responsibilities

  • Undertake health assessments and administer medications and treatments.
  • Run asthma, COPD, and diabetes clinics and reviews.
  • Manage long-term conditions and review medications for effectiveness.
  • Conduct cervical cytology and vaccinations; support immunisation programmes.
  • Perform phlebotomy, ECGs, wound care, and minor procedures as needed.
  • Promote health and well-being; develop patient-centred plans.
  • Support female sexual health, contraception and travel vaccinations.
  • Maintain patient records and ensure adherence to clinical protocols.

Job description

Ambrose Avenue Group Practice has a fantastic opportunity for an experienced practice nurse to join our team on either a full‑time or part‑time basis, with hours and days negotiable. The main responsibilities of the role include:

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met.
  • General health and well‑being promotion as well as creating individual, person‑centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long‑term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence‑based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.

All applicants should have the following qualifications, registrations, skills and experience:

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Ideally having completed the CNO002 Discovering a Nursing Career in General Practice module or the Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme), however we would still be interested in those applicants with equivalent experience or qualifications.
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long‑term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long‑term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
  • Immunisations - up to date.

Ambrose Avenue Group Practice is a busy general practice based across two sites (Ambrose Avenue and Tollgate) in Colchester. We are a large, dynamic team offering a stimulating and welcoming environment, caring for over 14,000 patients. We have 4 GP partners, 2 salaried GPs, 1 ANP, 2 nurse practitioners (soon to be ANPs), 1 diabetic nurse, 2 practice nurses, 2 HCAs, 1 H&W Coach, 1 GP assistant, 1 phlebotomist, 1 clinical pharmacist, 2 pharmacy technicians, and a wonderful team of administration staff. We use clinical systems such as SystmOne, Anima and Accurx and we are committed to training and developing both clinical and non‑clinical teams.
