Job description

We are looking for an experienced and enthusiastic Practice Nurse to join our dedicated team at Long Lane Surgery. This is a fantastic opportunity to work in a supportive and forward-thinking general practice committed to delivering high-quality, patient-centred care.

Main duties of the job

The Practice Nurse plays a key role in delivering high-quality, evidence-based care within the general practice team. Responsibilities include routine and chronic disease management (e.g. asthma, diabetes, hypertension), wound care, cervical cytology, immunisations, NHS Health Checks, and travel health advice. The nurse supports patients through health promotion, lifestyle advice, and opportunistic interventions.

Clinical duties include ECGs, BP checks, suture removal, administering medications under PGDs/PSDs, and maintaining accurate clinical records. The nurse collaborates with GPs, ANPs, and HCAs, and may assist in minor surgeries or act as a chaperone when needed.

The role includes ensuring the safe storage of vaccines, infection control, safeguarding awareness, and supporting practice protocols. Participation in audits, significant event reviews, and training is expected. The Practice Nurse must maintain NMC registration, engage in annual appraisal and CPD, and uphold professional standards, contributing to a safe and patient-centred environment.

Primary Responsibilities

Clinical Leadership

Act as lead nurse for Wound Care, Womens Health, and Anticoagulation, providing expertise and support across the team.

Deliver structured training to the nursing team in these lead areas, ensuring adherence to best practice and local/national guidance.

Chronic Disease Management

Oversee and participate in the delivery of chronic disease clinics (e.g. diabetes, asthma, COPD, hypertension).

Develop, implement, and evaluate individualised treatment plans for chronic conditions, in collaboration with the GP and wider team.

Monitor at-risk patients and implement early interventions to prevent deterioration in health.

General Clinical Duties

Undertake routine nursing tasks such as:

Blood pressure monitoring

ECGs

Suture and clip removal

Wound dressing and Doppler assessment

BMI measurement and lifestyle advice

Provide travel medicine advice and vaccinations, based on current guidance.

Implement vaccination programmes for adults and children under PGDs.

Carry out NHS Health Checks, including data recording and patient follow-up.

Women's Health

Perform cervical cytology (smear tests) according to national screening protocols.

Provide contraceptive advice and conduct annual reviews in line with FRSH guidance and practice policy.

Support patients in areas such as menopause management and reproductive health.

Emergency and Acute Care

Assist GPs with minor surgery procedures.

Support the team in clinical emergencies and perform anaphylaxis management in line with Resuscitation Council UK guidelines.

Provide clinical triage, assessing patients and referring or treating within scope of practice.

Mental Health and Safeguarding

Recognise and refer patients with mental health issues or safeguarding concerns.

Work within national and local safeguarding policies to protect vulnerable adults and children.

Liaise with social care and external agencies where appropriate.

Administrative and Governance Responsibilities

Clinical Records and Information Governance

Maintain accurate and timely clinical records in accordance with professional standards and data protection law.

Use read codes and clinical templates consistently to support patient safety and clinical effectiveness.

Contribute to the maintenance of chronic disease registers and patient recall systems.

Administer medications safely under Patient Group Directions (PGDs) or Patient Specific Directions (PSDs).

Monitor stock levels, ensuring the safe storage, rotation, and disposal of vaccines and medications.

Record cold chain data and report any discrepancies promptly.

Audit and Quality Improvement

Support clinical audit activities, including data collection, analysis, and action planning.

Contribute to practice improvement programmes by evaluating outcomes and suggesting improvements.

Participate in significant event reviews and near-miss investigations, using structured tools such as root cause analysis (RCA).

Education and Development

Participate in and contribute to mandatory training, clinical updates, and the practice training programme.

Act as a mentor or supervisor to junior nurses and students as required.

Engage in annual appraisal and revalidation processes, identifying personal learning needs and CPD opportunities.

Secondary Responsibilities

Contribute to policy and protocol development in collaboration with the clinical leadership team.

If qualified as an independent prescriber, prescribe within own competencies and in accordance with national guidance.

Participate in shared learning sessions and collaborative practice initiatives.

Support the implementation of Integrated Care Board (ICB) pathways and public health campaigns.

Monitor treatment room stock and assist with re-stocking other clinical areas when needed.

Participate in the planning and implementation of service changes and innovations.

Support the Deputy Practice Manager in staff inductions and orientation for new clinicians.
