We are ideally seeking someone to work 36 hoursover 4 days, and can offer some flexibility on working days.
Based at Witterings Medical Centre, a busy, wellestablished GP surgery looking after 10500 patients.
The practice nurse will play a crucial role inproviding high quality patient care and support within our medical centre.

Main duties of the job
Working as part of an established team to share responsibility for the implementation and provision of holistic care and health promotion to provide effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients as required.
Working as a member of the wider practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the local population.
The practice nurse will, as part of a team, be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

MAINDUTIES AND RESPONSIBILITIES
- The following are the core duties andresponsibilities to be undertaken by the post holder. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels.
- To support in developing, implementing and embeddinghealth promotion and disease prevention initiatives for the local population.
- To support in implementing and evaluatingindividual treatment plans for chronic disease patients that promote health andwellbeing.
- To support identifying, managing and empowering patientsat risk of developing long-term conditions, preventing adverse effects topatients health.
- To provide routine nursing care to patients inaccordance with clinical based evidence.
- In time to develop the skills to diagnose andmanage acute and chronic conditions.
- Provide wound care (including in time complexwound care such as ulcer/Doppler, etc.) to patients.
- To support in developing, implementing and embeddingan effective care management programme for the frail and housebound patientsregistered at the practice.
- To assist in provision of travel medicineservices.
- To request pathology services and processpathology results as required.
- As part of a team to oversee the provision ofchronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required.
- To maintain accurate clinical records inconjunction with extant legislation.
- To ensure SNOMED CT (READ) codes are usedeffectively by all members of the nursing team.
- To ensure the appropriate maintenance and use ofchronic disease registers.
- To ensure compliance with the practice chaperonepolicy.
- To assist GPs with minor surgery when required.
- To prioritise health issues and interveneappropriately.
- To support the team in dealing with clinicalemergencies.
- To recognise, assess and refer patientspresenting with mental health needs.
- To assist in implementing vaccination programmesfor adults and children.
- To support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice).
- To liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc).
- To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual).
- To support the clinical team with allsafeguarding matters, in accordance with local and national policies.
- To understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately.
- Develop, review and implement nursing protocolsin conjunction with the partners and lead nurse.
- Support all members of the nursing team,providing guidance when necessary, conducting appraisals and training needsanalyses as required.
- Participate in local initiatives to enhanceservice delivery and patient care.
- Support and participate in shared learning withinthe practice in order to improve patient care.
- Continually review clinical practices, respondingto national policies and initiatives where appropriate.
- Participate in the review of significant andnear-miss events applying a structured approach, i.e., root cause analysis (RCA).
- Promote a safe and clean environment in line withinfection control policies.
- Support the practice team with the reviewing andimplementation of practice policies and protocols, ensuring conformance toextant legislation.

Qualifications
- - Registered General Nurse Qualification
- - Membership of professional body
- - Additional qualifications in chronic disease management, sexual health, or other relevant specialisms
Experience
- - Previous experience in general practice or primary care
- - To work in accordance with NMC code of conduct
- - Ability to self-motivate, organise and prioritise workloads
- - Ability to develop in accordance with the needs of the practice
- - Competence in clinical skills such as phlebotomy, wound care, and injections.
- - Carry out risk assessment, surveillance and audit as required.
- - Ensure comprehensive written records are kept regarding any patients contact.
- - Ability to work independently and as part of a multidisciplinary team
- - To work flexible hours including some late clinics until 6.30pm.
- - Experience of TPP SystmOne
- - Experience/qualifications or a willingness to be trained in issues relating to health promotion, such as Asthma/COPD and Diabetes.
