Job Title: Practice Nurse Location: Great North Medical Group, Carcroft and Woodlands, Doncaster Job Summary An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our friendly and supportive team at Great North Medical Group. The successful candidate will provide high quality, evidence based nursing care to our practice population and work collaboratively with GPs, healthcare assistants and the wider multidisciplinary team to deliver safe, effective and patient centred services. The role will involve providing a range of treatment room and chronic disease management services, supporting health promotion initiatives and contributing to the ongoing development of clinical services within the practice. Main Duties and Responsibilities The successful candidate will be expected to: Deliver high quality nursing care within own scope of practice. Undertake chronic disease management reviews in line with practice protocols and national guidance. Provide health
promotion and lifestyle advice to patients. Administer vaccinations and immunisations in accordance with national and local guidance. Undertake cervical cytology screening. Carry out wound care, dressings and removal of sutures and clips. Perform routine treatment room procedures including blood pressure monitoring, ECGs, venepuncture and other relevant clinical interventions. Support infection prevention and control procedures. Maintain accurate, contemporaneous and complete clinical records. Participate in clinical audits, quality improvement initiatives and practice meetings. Work collaboratively with the wider multidisciplinary team to support patient care. Identify and report safeguarding concerns in accordance with practice policies. Contribute to the achievement of Quality and Outcomes Framework (QOF) targets and other contractual requirements. Maintain professional registration and undertake continuing professional development.
