Main duties of the job
Job Responsibilities
- Deliver practice nursing services within NMC Code of Conduct and agreed Practice standards.
- Manage patients with chronic diseases, following quality frameworks and evidence-based guidelines.
- Contribute to the development of integrated services within Primary Care.
- Oversee infection control, ensuring compliance.
- Participate in annual appraisals.
- Perform clinical duties, including chronic disease management, cervical screening, immunisations, and lifestyle advice.
- Recognise signs of abuse and work with safeguarding teams where relevant.
- Maintain accurate records, participate in audits, and update disease registers.
- Establish good working relationships and work well within a multidisciplinary team.
- Work both independently and collaboratively within a multidisciplinary team.
- Demonstrate strong organisational and planning skills, able to work under pressure.
- Exhibit excellent communication skills both verbally and written.
- Have experience in dealing with the public/patients.
- Be willing to develop nursing skills and adapt easily to a changing environment.
- Possess a good level of IT skills.
- Follow instructions and use own initiative.

Our well-established and well-respected practice is currently operating across three sites: Elizabeth Street Surgery, Gorton Street Surgery, and Harris Medical Centre. We are temporarily working from Elizabeth Street and Gorton Street until our new build on Adelaide Street is ready.

Job responsibilities
- Work within NMC Code of Conduct and practice standards and protocols.
- Effectively manage patients with Chronic Diseases as per quality schemes.
- Support implementation of evidence-based guidelines.
- Ensure compliance with Care Quality Commission indicators, demonstrating evidence, review, and audit cycles.
- Participate in the practice appraisal process and develop a personal development plan.
- Engage in multidisciplinary clinical and practice meetings.
- Support and develop clinical and practice protocols.
- Perform clinical nursing duties such as managing chronic diseases, immunisations, venepuncture, and electrocardiography.
- Support patients in engaging in healthier lifestyles and provide travel health care.
- Maintain and update disease registers.
- Promote a learning culture and support clinical training and supervision.
- Establish effective communication pathways within the practice and with external teams.
Person Specification
Experience
- Registered General Nurse (diploma or degree), NMC registration, and a clean driver’s license.
- Practice Nurse qualification/experience, with expertise in chronic disease management, emergency resuscitation, and familiarity with EMIS Clinical System.
- Special interest in respiratory medicine preferred.
