Job summary

GP Practice in Wales is seeking an experienced Practice Nurse to join a friendly, supportive team. The role runs 30-37.5 hours per week, with flexibility for the right candidate. Essential experience includes general practice, cervical screening, and chronic disease management.

Qualifications

• Experience in General Practice essential; cervical screening and chronic disease management skills required.
• Chronic Disease Management experience advantageous; Older Adult Vaccinations and Wound Management desirable; QAIF understanding essential.

Responsibilities

• Assist in routine patient care tasks as directed by Nurse Lead and GPs.
• Perform venepuncture and cytology.
• Administer adult vaccinations and help manage vaccination stock.
• Coordinate LTC management for chronic diseases (e.g., diabetes, CHD, hypertension, asthma, stroke/CVA, COPD).
• Order pathology tests and ensure accurate record-keeping.
• Maintain infection control and participate in infection control audits.

Job Responsibilities

• Assist in and perform routine tasks related to patient care as directed by Nurse Lead and GPs
• Cytology
• Venepuncture
• Adult Vaccinations
• Requesting pathology tests, for example urine culture, swabs
• LTC management including Diabetes, Coronary Heart Disease, Hypertension, Asthma, CVA, COPD
• Ordering of vaccinations to maintain stock levels
• Ability to deliver COVID Vaccinations
• Following agreed clinical protocols with referral from GPs as appropriate
• Awareness of infection control and requirement to take part in infection control audit
• Participation in administration systems in the Practice (EMIS and Docman)
• At all times there will be a need to maintain accurate records
• A duty to advise GPs of potential problems or errors within a range of assigned tasks
• Attend and participate in any Nurse and Practice meetings when required
• Any other delegated duties appropriate to the post

We are looking for an experienced and enthusiastic Practice Nurse to join our friendly and supportive team.

The role is for 30-37.5 hours per week (flexible for the right candidate). Previous General Practice experience is essential, with skills in cervical screening and chronic disease management. Chronic Disease Management would be an advantage but Older Adult Vaccinations, Wound Management and an understanding of QAIF are essential requirements for this role.

Desirable skills include Independent Prescribing Minor Illness Management.
