Job summary

Thera Trust is seeking a Practice Leader to mentor Support Workers and drive person-centred support for people with learning disabilities. You will help individuals exercise choice and control in planning and delivery, while promoting community inclusion and participation.

The role emphasizes improving outcomes, safeguarding, and maintaining a safe, supportive environment. You will work toward a Level 3 Diploma in Health and Social Care and may travel locally to multiple locations.

Qualifications

• Experience supporting people with learning disabilities.
• Knowledge of safeguarding, health and safety, and care regulations.
• Excellent communication, leadership, organisational, and decision-making skills.
• Confidence in using digital systems (e.g. Excel, care planning software).
• Flexible working hours, willingness to travel locally and work across multiple locations.

Responsibilities

• Mentor Support Workers to improve outcomes and promote person-centred support.
• Focus on ensuring choice and control in delivery and maintaining a safe environment.
• Support individuals with personal care, finances, meals, and community access.
• Promote emotional and psychological well-being and continuous improvement through feedback.
• Act as an ambassador for the company, upholding policies and inclusion.
