Job summary

Rotherham Doncaster & South Humber NHS Foundation Trust is seeking a PBS Practitioner to lead specialist assessment and intervention for individuals with learning disabilities and autism whose behaviours of concern impact quality of life.

You will develop and review Positive Behaviour Support plans, provide expert PBS guidance, and support families, carers and professionals to achieve improved outcomes through person-centred practice within an MDT setting.

Responsibilities

• Complete specialist functional assessments and formulations for people with learning disabilities and autism who present with behaviours of concern.
• Develop, implement and review Positive Behaviour Support plans in partnership with individuals, families, carers and professionals.
• Provide specialist PBS advice, guidance and consultation to colleagues, support providers and partner agencies.
• Manage a defined caseload and contribute to holistic assessment, intervention and care planning within the MDT.
• Deliver PBS-related training, coaching and support to staff, carers and other stakeholders.
• Promote trauma-informed, evidence-based and person-centred approaches in all aspects of practice.
• Contribute to service development, clinical governance, quality improvement and audit activities.
• Maintain accurate clinical records and ensure compliance with safeguarding, confidentiality and professional standards.
