Job summary

Knowsley Metropolitan Borough Council is seeking a Portage Practitioner within the Knowsley Early Years Service to support pre-school children with additional needs using the Portage Model.

You will provide Educational Home Visiting, lead Early Help Assessments, contribute to Education, Health and Care Plans, and work with parents, carers and partners across the Local Authority and Health sectors to raise educational outcomes.

Qualifications

• Childcare qualification equivalent to NVQ level 3 or higher.
• Evidence of continued professional development.
• Complete Basic Portage workshop, core competencies and 6 months supervised home visits.

Responsibilities

• Provide Educational Home Visiting for pre-school children with additional needs and/or disabilities.
• Lead the Early Help Assessments and contribute to Education, Health and Care Plans.
• Assist the Early Years SEND Senior Practitioner with regular reviews of families receiving a Portage Service.
• Deliver Portage groups for children and their families on the waiting list.
• Liaise with Local Authority, Health, Voluntary Sector and other stakeholders to improve educational outcomes.
