Sanctuary Personnel is seeking a Physiotherapy Assistant in Newham, London, for a 3-month contract. The role focuses on delivering rehabilitation and reablement in a community setting, collaborating with physiotherapists and occupational therapists.

Hybrid working with 1–2 days onsite, 36 hours weekly, and a salary up to £25 per hour. The post involves supporting adults to maximise independence, assisting with assessments, monitoring outcomes, and managing records in electronic systems.

Qualifications

• Experience working within a Local Authority.
• Experience in Rehabilitation and Reablement.
• Experience supporting adults to achieve rehabilitation goals.
• Experience maintaining electronic records.
• Knowledge of the Care Act 2014 and Reablement pathways.
• Community-based rehabilitation experience.
• Experience working within multidisciplinary teams.

Responsibilities

• Support the delivery of rehabilitation and reablement programmes for adults living in the community.
• Work alongside Reablement Officers, Physiotherapists, Occupational Therapists, and other health and social care professionals.
• Assist with assessments, reviews, and outcome monitoring.
• Support service users to maximise independence and achieve agreed rehabilitation goals.
• Assist with equipment provision and assistive technology interventions.
• Maintain accurate records using electronic care management systems.
• Collaborate with health, social care, and third-sector partners to support ongoing care needs.
