What you will do:

  1. Collaborate effectively as part of an integrated team to deliver top-notch therapeutic care, helping patients achieve maximum independence in their daily lives and actively promoting self-management.
  2. Provide highly specialist assessment, treatment, and person-centred management for patients in need of community rehabilitation, ensuring high standards of care.
  3. Lead and further develop the community rehabilitation team service, ensuring seamless planning, coordination, and evaluation of the service provided.
  4. Mentor and develop team members, empowering them to deliver optimal care to individuals residing in the community.
  5. Collaborate with allied services including community nursing, specialist nurse and therapy services, palliative care professionals, adult social care, and the voluntary sector. Your teamwork will help develop effective care pathways for people with rehabilitation needs.

Requirements:

  1. HCPC registration is required to ensure high standards of professional practice.
  2. At least two years of experience in a relevant specialism will be invaluable.
  3. A car driver is preferable to facilitate travel between community sites.
