Job summary

SJB Medical is recruiting a Band 5 Physiotherapist to join a multidisciplinary rehabilitation team in a specialist intermediate care service in Ware, Hertfordshire. You will manage your own caseload, develop tailored treatment plans, and contribute to discharge planning and community transitions.

The role focuses on restoring mobility and independence for older adults after hospital discharge, with opportunities for professional development and working within a supportive team environment.

Qualifications

• Degree in Physiotherapy.
• Current HCPC registration.
• Strong understanding of professional standards and physiotherapy best practice.
• Excellent communication and interpersonal skills.
• Experience with older adults or rehabilitation services is desirable but not essential.

Responsibilities

• Manage a caseload of residents requiring rehabilitation support.
• Conduct comprehensive physiotherapy assessments and develop personalised treatment plans.
• Deliver evidence-based rehabilitation programmes focused on mobility, strength, and independence.
• Work alongside residents to establish meaningful and achievable rehabilitation goals.
• Support discharge planning and safe transitions back into the community.
• Collaborate with Occupational Therapists, Social Workers, Doctors, and wider healthcare teams.
• Provide guidance and supervision to therapy assistants.
• Participate in multidisciplinary meetings and case reviews.
