Job summary

Cpl UK Healthcare is recruiting a Band 6 Physiotherapist to join a Wirral-based service as a Falls Practitioner in the community. You will assess and rehabilitate patients at risk of falls, promote safety, and support prevention strategies to reduce hospital admissions.

You will work within community settings, collaborating with the multidisciplinary team and maintaining records and outcome measures with HCPC registered practice and strong communication skills.

Qualifications

• HCPC registration is required.
• Experience in physiotherapy, ideally in community, elderly care, or falls prevention.
• Strong mobility, balance, and rehabilitation knowledge.

Responsibilities

• Manage a caseload of patients at risk of falls in the community.
• Complete comprehensive falls risk and mobility assessments.
• Develop and implement individualised rehabilitation and prevention plans.
