Join a leading health service as a Physiotherapist in Sunderland, delivering high-quality rehabilitation therapy to support adults recovering from illness or trauma. Physiotherapists play a key role in the service, completing functional assessments and working with people to co-produce and implement person-centred rehabilitation therapy goal plans.
Supervision will be provided by a Therapy Team Manager or Senior Practitioner in the Community Rehabilitation Service.
The service strives to maintain high standards of customer care and works with its practitioners to assure continuous professional development, demonstrating best practice and a high commitment to the undergraduate education of Physiotherapy and Occupational Therapy students.
Candidates should have the ability to provide safe and effective Physiotherapy assessment and intervention. They should also be able to work collaboratively and have effective communication and listening skills, including skills to produce good quality reports.
Essential requirements are a diploma/degree in Physiotherapy & current registration with the Health & Care Professions Council.
Therapies work Monday to Friday, and the council offers flexible working patterns, including a combination of office-based and home working.
The Community Therapy Service at Sunderland City Council is a thriving provider of a range of rehabilitation-based services across the city. The Community Rehabilitation Team is an integral part of the service and has an exciting opportunity for a Physiotherapist to join the team.
The Community Rehabilitation Team delivers rehabilitation to customers living in their own homes or engaging in an intermediate care journey. The team consists of Physiotherapists, Occupational Therapists, and Rehabilitation Therapy Assistants, working in collaboration with health, social care, and voluntary sector providers via a multidisciplinary approach.
The team is dynamic, supportive, and responsive, with a primary objective of supporting residents in managing their disabilities or long-term health conditions, enabling them to lead fulfilling, safe, and independent lives.
Main purpose of job
To assist in the efficient provision of high-quality Therapy services to meet the needs of adults recovering from illness, trauma, or decline in long-term conditions, or experiencing effects of aging.
Key responsibilities
To carry an allocated caseload of customers and provide assessments in line with the Care Act 2014.
To support customers to achieve recovery and rehabilitation in various settings, including community bed-based services, care homes, and customers' own homes.
To provide holistic assessments of customers, identifying needs and ensuring appropriate provision to improve quality of life.
To work to set standards within the service under the supervision of the Senior Practitioner/Team Manager.
To complete assessments of customer needs, including functional assessments, assistive equipment provision, and rehabilitation.
To be adaptable and flexible within service areas, working alongside senior therapists to ensure effective service delivery.
To collaborate with company representatives for assessments of specialist or bespoke equipment.
If required, to participate in the Emergency Response rota to stabilize customers where circumstances have changed.
To work alongside Occupational Therapy & Physiotherapy colleagues, Senior Practitioners, and the Team Manager to deliver a safe, high-quality service.
To demonstrate the ability to conduct appropriate assessments for customers.
To advise customers on the correct use of equipment provided by the Local Authority.
To undertake assessments for standard wheelchair provision, with some enhanced prescribing rights as directed by the Team Manager.
To collaborate with other services and refer customers to them as needed.
To record all assessments and be responsible for data input.
To recognize limitations and seek support from line managers when necessary.
To participate in project activities within the service under the guidance of the Senior Practitioner/Team Manager.
To identify learning needs and areas for development, participating in training and educational opportunities.
To participate in supervision and appraisal with line managers.
To maintain professional registration with the Health & Care Professions Council and up-to-date Disclosure and Barring Scheme checks.
The post holder must carry out duties in accordance with the Council's policies on equal opportunities, health and safety, data protection, and confidentiality, including compliance with GDPR and the Data Protection Act 2018.
This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary: £36,124 to £44,711 per year, pro-rata - 18.5 hours role.
