At CLCH, we proudly serve as a beacon of compassionate community healthcare across 14 vibrant London boroughs — Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster, Hillingdon, Sutton, and Kingston — as well as Hertfordshire. Job overview

Join our integrated Urgent Community Response and Hospital at Home services as a Physiotherapist, providing high-quality, autonomous assessment and treatment for people with acute illness, frailty, functional deterioration and complex health needs in community settings. This is a part time vacancy, 18.75 hours per week.

Using specialist clinical assessment, reasoning and rehabilitation skills, you will support people with complex mobility, falls, respiratory, neurological and musculoskeletal needs. Your work will help prevent hospital admissions, support Hospital at Home care, facilitate early supported discharge and promote independence through evidence-based, person-centred interventions.

You will manage an allocated caseload, contribute to clinical triage and prioritisation, identify deterioration and elevate appropriately. Working closely with multidisciplinary colleagues across primary, community, acute and social care, you will help deliver safe, coordinated care.

The role includes regular home visits, transporting equipment and safe manual handling, plus contributions to supervision, staff development and quality improvement.

Rotational shifts are between 08:00 and 23:00, seven days a week. A full valid UK driving licence and access to a car for business use are required, unless you have a disability as defined by the Equality Act 2010.

Main duties of the job

You will provide skilled, effective and evidence-based physiotherapy care to patients in their homes and other community settings.

Your main duties will include:

  • Undertaking face-to-face clinical assessments and interventions, including transporting and handling clinical equipment safely in environments with varying physical and accessibility constraints.
  • Assessing and triaging referrals, prioritising and responding according to clinical urgency and agreed timescales, and seeking senior support for cases outside your competence.
  • Completing comprehensive physiotherapy assessments covering clinical history, observations, functional mobility, transfers, balance, gait, falls risk, frailty, respiratory function, neurological and musculoskeletal presentations, cognition and relevant psychosocial factors.
  • Applying clinical reasoning to identify presenting problems, formulate physiotherapy diagnoses and rehabilitation goals, and deliver evidence-based, person-centred interventions.
  • Recognising red flags and clinical deterioration and escalating promptly to appropriate senior, medical or emergency services.
  • Working autonomously within your professional competence while collaborating with multidisciplinary colleagues to support safe, coordinated care.
Working for our organisation

We are proud to be one of the largest community healthcare providers in the country, with more than 4,500 colleagues caring for over four million people across London and Hertfordshire. Every day, our teams bring their skill, compassion, and determination to the people who depend on us.

What inspires us is at the heart of who we are: when we work together, we can help people move forward in ways that truly matter. Our teams support children as they take their first steps in life, and they stand beside adults as they rebuild strength, confidence, and independence. From newborn health visiting to community nursing, stroke rehabilitation, and palliative care, we are there for people through some of life’s most important moments.

Joining Central London Community Healthcare means becoming part of a community that lifts each other up. It means working in an organisation that values compassion, welcomes new ideas, and believes in the potential of every colleague. Your development matters here. Your wellbeing matters. Your voice helps shape the future of the care we provide.

We offer a competitive employment package because the work you do matters. At Central London Community Healthcare, you will join an inclusive organisation that invests in its people, supports development, and helps you thrive while delivering high-quality care.
