Job overview

Join our integrated Urgent Community Response and Hospital at Home services as a Physiotherapist, providing high-quality, autonomous assessment and treatment for people with acute illness, frailty, functional deterioration and complex health needs in community settings.

Using specialist clinical assessment, reasoning and rehabilitation skills, you will support people with complex mobility, falls, respiratory, neurological and musculoskeletal needs. Your work will help prevent hospital admissions, support Hospital at Home care, facilitate early supported discharge and promote independence through evidence-based, person-centred interventions.

You will manage an allocated caseload, contribute to clinical triage and prioritisation, identify deterioration and elevate concerns appropriately. Working closely with multidisciplinary colleagues across primary, community, acute and social care, you will help deliver safe, coordinated care.

The role includes regular home visits, transporting equipment and safe manual handling, plus contributions to supervision, staff development and quality improvement.

Rotational shifts are between 08:00 and 23:00, seven days a week. A full valid UK driving licence and access to a car for business use are required, unless you have a disability as defined by the Equality Act 2010.

Main duties of the job

You will provide skilled, effective and evidence-based physiotherapy care to patients in their homes and other community settings.

  • Undertaking face-to-face clinical assessments and interventions, including transporting and handling clinical equipment safely in environments with varying physical and accessibility constraints.
  • Assessing and triaging referrals, prioritising and responding according to clinical urgency and agreed timescales, and seeking senior support for cases outside your competence.
  • Completing comprehensive physiotherapy assessments covering clinical history, observations, functional mobility, transfers, balance, gait, falls risk, frailty, respiratory function, neurological and musculoskeletal presentations, cognition and relevant psychosocial factors.
  • Applying clinical reasoning to identify presenting problems, formulate physiotherapy diagnoses and rehabilitation goals, and deliver evidence-based, person-centred interventions.
  • Recognising red flags and clinical deterioration and escalating promptly to appropriate senior, medical or emergency services.
  • Working autonomously within your professional competence while collaborating with multidisciplinary colleagues to support safe, coordinated care.
