Job description

An exciting opportunity for a 12 month fixed term/secondment has arisen for an enthusiastic and motivated Band 5 Physiotherapist to join our newly established Integrated Neighbourhood Team (INT). This is a unique opportunity to be part of an innovative model of care that brings together health and social care professionals to deliver more joined-up, person-centred support within local communities.

Working as part of a multidisciplinary team, you will provide physiotherapy assessment, treatment and rehabilitation for people with a wide range of conditions and needs across all age groups. You will support individuals to maximise their independence, improve function, and achieve goals that matter most to them.

This role offers the opportunity to work across a diverse caseload including reablement, admission avoidance, proactive care, rehabilitation, and the management of long-term conditions. You will work collaboratively with colleagues from a variety of professions to deliver coordinated care that keeps people well, independent, and supported within their own homes wherever possible.

Please be advised as this is a community position, being a car driver with a valid driving license for the UK and access to a car with business insurance is essential.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

As a Band 5 Physiotherapist within the Integrated Neighbourhood Team, you will:

  • Assess, plan, implement and evaluate physiotherapy interventions for patients within community settings.
  • Deliver rehabilitation and reablement programmes to support independence and recovery.
  • Support admission avoidance initiatives by providing timely assessment and intervention.
  • Contribute to proactive care planning for individuals with complex and long-term conditions.
  • Develop and review personalised goals with patients, carers and families, supporting progression towards agreed outcomes.
  • Work closely with multidisciplinary colleagues including occupational therapists, nurses, social workers, support workers, paramedics, GPs and other healthcare professionals.
  • Promote self-management, health promotion and preventative approaches to care.
  • Maintain accurate clinical records and contribute to service development and quality improvement initiatives.
  • Participate in team meetings, case discussions and collaborative care planning.
