This post is offered on a 12-month fixed-term basis to cover maternity leave.
You will deliver a high-quality physiotherapy service to patients within the community hospital, providing assessment, treatment and rehabilitation to support patients in achieving their optimum level of independence.
You will assess, plan, implement and evaluate patient care programmes, using evidence-based practice to meet the rehabilitation needs of patients. You will adopt a holistic, patient-centred approach, ensuring patients and carers are actively involved in decision-making and goal setting.
You will contribute to effective discharge planning and work collaboratively with colleagues across health and social care to facilitate safe and timely discharges, supporting the reduction of unnecessary inpatient length of stay where appropriate.
You will also provide physiotherapy advice and support to ward staff, patients, carers and partner organisations, promoting effective communication and continuity of care to achieve the best possible outcomes for patients.
