Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Band 5 Physiotherapist to join the Integrated Neighbourhood Team on a 12-month fixed term/secondment basis. The role focuses on community-based physiotherapy, working across diverse conditions and age groups to maximise independence and function.

You will assess, plan and deliver rehabilitation, collaborate with a multidisciplinary team, and contribute to proactive care planning while promoting self-management and prevention

Responsibilities

• Assess, plan, implement and evaluate physiotherapy interventions for patients within community settings.
• Deliver rehabilitation and reablement programmes to support independence and recovery.
• Support admission avoidance initiatives by providing timely assessment and intervention.
• Contribute to proactive care planning for individuals with complex and long-term conditions.
• Develop and review personalised goals with patients, carers and families, supporting progression towards agreed outcomes.
• Work closely with multidisciplinary colleagues including occupational therapists, nurses, social workers, support workers, paramedics, GPs and other healthcare professionals.
• Promote self-management, health promotion and preventative approaches to care.
• Maintain accurate clinical records and contribute to service development and quality improvement initiatives.
• Participate in team meetings, case discussions and collaborative care planning.
