Job summary

Cardiff and Vale University Health Board seeks a Primary Care Physician Associate to join our community team to deliver acute and chronic disease management with supervision from a GP. The role supports frailty management and works within a multidisciplinary team to provide high-quality patient care.

You will manage clinical caseload, review diagnostic results, support patients in prescribed medicines and engage in audits and service development within the PCN framework.

Qualifications

  • GMC Registration and UK Registration as a Physician Associate.

Job description

We are seeking a Primary Care Physician Associate to join our team to deliver innovative care to the community. We are looking for a Physician Associate who can act within their professional boundaries to deliver acute and chronic disease management and will look to develop confidence in being part of the multidisciplinary team delivering visits and frailty management. Appropriate supervision and support will be available to Physician Associates. In addition, the PCN offers dedicated CBP time along with peer to peer support and mentorship/Main duties of the job Work within scope of practice under the supervision of a GP to deliver high levels of patient care using advanced, theoretical knowledge, autonomous clinical skills and evidence based practice. Manage clinical case load and address patient needs. Review, analyse and action diagnostic test results. Collect/request pathology specimens as required. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice). Deliver integrated patient centred-care through appropriate working with the wider multi-disciplinary team and social care networks. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times. Ensure continuity of care, arranging follow-up consultations or reviews as necessary. Partake in clinical audits, significant event reviews and other research and analysis tasks., The following are the core responsibilities of the Physician Associate. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels. The Physician Associate will work within their scope of clinical practice and: Work within scope of practice under the supervision of a GP to deliver high levels of patient care using advanced, theoretical knowledge, autonomous clinical skills and evidence-based practice. Manage clinical case load and address patient needs. Review, analyse and action diagnostic test results. Collect/request pathology specimens as required. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice). Deliver integrated patient centred-care through appropriate working with the wider multi-disciplinary team and social care networks. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times. Ensure continuity of care, arranging follow-up consultations or reviews as necessary. Partake in clinical audits, significant event reviews and other research and analysis tasks. Play an active role in the management of patients with long term conditions. Deliver opportunistic health promotion where appropriate. Liaise with external services and agencies to ensure that the patient is supported appropriately (vulnerable patients etc). Support the clinical team with all safeguarding matters, in accordance with local and national policies. Carry out immunisations as required. Maintain accurate clinical records in conjunction with current legislation. Ensure read codes are used effectively. Work within scope of practice, and seek assistance and support when required. Contribute to regular practice meetings.
