Full-Time Physical Therapist Needed in Warwick, New York

Job Type: Full-Time / Part-Time

Salary: $110,000 - $125,000 per year + benefits

Position Overview:

We are currently seeking a Licensed Physical Therapist (PT) to join our dynamic rehab team and help lead the way in delivering outstanding, individualized therapy services to our residents.

As a Physical Therapist in our skilled nursing facility, you will evaluate residents, create personalized treatment plans, and provide interventions that support recovery and improve functional outcomes. You’ll work closely with a team of PTAs, OTs, SLPs, nurses, and physicians to ensure residents receive coordinated, high-quality rehabilitative care.

Job Duties:

  1. Conduct thorough evaluations and assessments of residents’ physical capabilities and limitations
  2. Develop, implement, and modify individualized treatment plans to meet specific functional goals
  3. Provide direct therapy services, including therapeutic exercise, mobility training, pain management, and education
  4. Monitor and document progress toward treatment goals and adjust plans as needed
  5. Maintain accurate and timely documentation in the facility’s electronic health record (EHR)
  6. Collaborate with PTAs and interdisciplinary team members for comprehensive resident care
  7. Educate residents, families, and caregivers on exercises, safety, and home care techniques when appropriate

Minimum Requirements:

  1. Active New York State Physical Therapist license (required)
  2. Graduate of an accredited Physical Therapy program (DPT, MPT, or equivalent)
  3. Current CPR/BLS certification
  4. Strong communication and clinical decision-making skills
  5. Compassionate, professional demeanor with a commitment to resident-centered care
  6. Ability to work effectively in a fast-paced SNF environment
  7. Prior experience in a Skilled Nursing Facility or post-acute rehabilitation setting
  8. Familiarity with Medicare guidelines and PDPM documentation requirements
  9. Proficiency with electronic documentation systems (e.g., PointClickCare, NetHealth, Rehab Optima)
  10. Experience with balance training, fall prevention, orthopedic and neurorehabilitation
