Job summary

Sentara Healthcare Inc is seeking a qualified Physical Therapist to assess, plan, and implement rehabilitative programs that improve mobility and reduce pain. You will educate patients and caregivers and supervise care provided by the rehabilitation team.

In this role, you will collaborate with internal and external clinicians, tailor plans of care, and ensure evidence-based practice is followed to optimize patient outcomes across ages and conditions.

Qualifications

• Bachelor's degree in physical therapy is required.
• Master's degree is preferred.
• Doctorate in physical therapy is an option.
• Licensure as a physical therapist is required.

Responsibilities

• Performs modalities/therapeutic interventions in line with standards of care and department policies.
• Provides patient and family education through standardized programs or individualized training.
• Initiates and maintains collaborative interactions with internal/external team members to improve patient outcomes.
• Selects and performs appropriate assessment procedures and updates formal care plans.

Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members

• Selects and performs the appropriate assessment procedures. Establishes and updates formal plan of care to address individual patient needs.
