Duke Medical Centre in Sheffield is seeking an experienced pharmacist to cover maternity leave on a fixed-term contract, delivering 24 hours per week. You will collaborate with GPs and a multidisciplinary team to optimise patient care and medicines management.
The role focuses on clinical medication reviews, safer prescribing, and providing medicines advice to staff and patients within a busy general practice setting.
We are looking for an experienced clinical pharmacist to join our busy practice. You will be helping to cover maternity leave on a Fixed term contract and supporting the practice as part of Pilot initiative, covering 24 hours per week. You will be working along-side our multi-disciplinary team to deliver the best healthcare service for our patients.
As a Pharmacist you will be responsible for working alongside GPs and other member of the clinical team dealing with patients' clinical medication reviews, including those with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system; deal with acute prescription requests. Furthermore, they will promote systems for safer prescribing and provide expertise in clinical medicines advice.
Our practice is based in the park hill area of Sheffield. We have 4 GP Partners and 2 Salaried GP's. Our nursing team consists of, 1 pharmacist, 2 Practice Nurses and 1 Health Care Assistants and care coordinator.
We are part of the GPA1 PCN which consists of the 5 local practices which share variety of services including a physiotherapist and Mental Health Team.
