Job summary

The Pharmacist Network are recruiting an onsite IP Clinical Pharmacist to join a partner general practice in Salford. You will manage day-to-day clinical pharmacy activities and provide pharmaceutical care to patients in a busy primary care setting.

Requirements include a minimum of 12 months in general practice, GP pharmacy registration with GPhC, independent prescribing, and experience with EMIS or System One. Commutable to Salford is expected.

Qualifications

  • Minimum 12 months' experience in general practice/primary care.
  • GPhC registered with independent prescribing qualification.
  • IP qualification or close to completion.
  • Experience with EMIS or System One.
  • CPPE Primary Care pathway completed or in progress.
  • Willing to commute to onsite location.

Responsibilities

  • Provide medication-related advice and resolve queries as part of a multidisciplinary team.
  • Conduct polypharmacy reviews and SMRs; perform face-to-face reviews.
  • Support medicines optimisation and safe use of medications within the practice.
  • Reconcile medications and review post-discharge where applicable.
  • Carry out audits and collaborate on service improvements with the practice.
  • Provide expert clinical leadership to pharmacists and technicians.

Our senior clinical team support the training, development and workflow management of Pharmacists and Pharmacy Technicians within primary care and PCN settings.

Main duties of the job

We currently have an exciting opportunity for an IP Clinical Pharmacist, to work full time for our partner practice within Salford, this opportunity is available for highly motivated and focused Clinical Pharmacist, who will be responsible for the smooth running of day-to-day activities within general practice, while playing a vital role in providing pharmaceutical care and advice to patients focusing on best practice and adhering to safety guidelines.

Job responsibilities
The Role:

As an onsite Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.

You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.

  • Help to utilise skill mix

  • Support practice staff with prescription and medication queries
  • Medicine reconciliation and review post-discharge
  • Conduct medical related audits and work on service improvement with the practice
  • Provide expert advice on medications to other clinicians and patients
  • Deliver clinical leadership on medicines optimisation and quality improvement
  • Ensure appropriate and efficient use of medication
  • Educate patients on medication and disease prevention
  • An understanding of QOF, CQC and local incentive scheme workstream
  • Create and follow clinical pharmacy programme according to practice policy
Job Requirements:
  • A minimum of 12 months experience of working within general practice/primary care
  • A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
  • Independent prescribing qualification
  • Experience with EMIS or System 1
  • Completed the CPPE Primary Care pathway or close to completion
  • Able to commute to onsite location / live within close proximity of practice
  • Excellent communication and interpersonal skills
