Additional hours may occasionally be required to cover holiday.We are a small team that provide high-quality support for an individual with physical and mental learning disabilities, enabling her to live safely, comfortably, and with dignity in her own home.Support focuses on:Personal careHealth and medical needsDaily living activities, including personal care, cooking, social activities, providing engaging activities at home.Ensure the home environment is clean, safe, and comfortable.Key Responsibilities:Follow individual care plans, risk assessments, and health protocols.Assist with personal hygiene, and dressing.Support with medicationPromote independence and choice wherever possible.Encourage individual to engage in preferred activities and community outingsRecord health information, behaviour patterns and report concerns promptly to the Team Leader.1.Behavioral & Emotional SupportProvide reassurance and calming strategies when the person experiences anxiety or distress.Recognise behavioural cues and respond appropriately using positive behaviour support techniques.Encourage engagement in preferred activities.2. Communication & EngagementUse knowledge of clients verbal communication and behavioural cues to meet her needs.Support social inclusion and community access, while managing risks and triggers.Promote dignity, respect, and choice at all times.3. Safeguarding & Legal ComplianceWork in line with the Care Act 2014, Mental Capacity Act 2005, and Equality Act 2010.Safeguard the individual from harm, abuse, or neglect.Maintain confidentiality4.
