Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a Perinatal Practitioner (OT, Social Worker or RMN) to cover the East team at Horizon House in Epsom. You will work within a multi-disciplinary team to assess, plan, implement and evaluate nursing care for pregnant and postnatal women and their families.

You will mentor and supervise staff including students, manage a defined caseload and coordinate triage and referrals.

Qualifications

• Must be a Perinatal Practitioner (OT, Social Worker or RMN)
• Will assess, plan, implement and evaluate nursing care for pregnant/postnatal women
• Mentor, supervise and teach qualified and unqualified staff including students

Responsibilities

• Be accountable for a defined caseload and assess care needs under supervision of the Clinical Lead Nurse
• Manage referrals with appropriate triage and timely assessments across the service
• Respond to emergency referrals, support referrals within the acute care pathway and liaise with HTT, inpatient services and MBU
• Participate in multi-disciplinary assessment of women with perinatal mental health needs in the community
