Responsabilidades

• Maintain a clinical caseload and carry out all Care Coordinator roles and responsibilities.
• Provide high quality mental health assessments and treatment programmes within the multidisciplinary team.
• Manage risk for patients.
• Deliver evidence-based interventions and recovery-focused work with patients and carers.
• Visit people in their own homes and communities.
• Run clinics at the team base and other community venues.
• Work autonomously as well as within the MDT and other settings.
