Job summary

Meeveem Limited is seeking a specialist paramedic to join the Wells Health Centre team within the West Mendip PCN. The post involves autonomous clinical practice, collaboration with GPs and the multi-disciplinary team, and delivering holistic care including assessment, diagnosis and treatment of minor illnesses or injuries.

The role supports education for patients, self-care, and effective triage, with duties spanning clinical and administrative tasks.

Responsibilities

  • Assess and triage patients, using same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the Practice, outside agencies and the PCN team.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
  • Be able to: Utilise triage knowledge and skills to appropriately direct patients to the correct care in-house or to the appropriate service. Perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.

  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
  • Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care.
  • Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
  • Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Job summary

The role will provide a specialist paramedic resource for healthcare professionals and service users, working in collaboration with other members of the multi-disciplinary team, working within the surgery and the wider PCN as an autonomous, accountable paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.This is a full-time role (37.5 hours per week). Part-time working may be considered in line with service requirements. This role is included within the extended hours provision and may involve working an evening and/or Saturday on a rota basis.

Main duties of the job

The post-holder will provide clinical expertise as an integral member of the Wells Health Centre multidisciplinary team, to assess, diagnose, treat, refer or signpost patients/service users, who attend the surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

A paramedic is authorised to undertake treatments and procedures as directed by the GPs for which they are trained in, and which are within the Royal College of Paramedics guidelines and in line with the HCPC standards framework. The post holder will use advanced clinical skills to provide education to patients and service users, promoting selfcare and empowering them to make informed choices about their treatment.

The post-holder will also perform any other clinical and administrative duties, where trained and determined safe to do so by the partners. This may include assisting with medical reports, safeguarding, and clinical support.

Through these activities, the post-holder will support the delivery and development of high-quality care within Wells Health Centre while contributing, where appropriate, to relevant West Mendip PCN priorities and services.

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.

One of 13 PCNs in Somerset, we are a collaboration of five GP practices in West Mendip: Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice, Wells Health Centre and Vine Surgery Partnership (Street).
