Job summary

The Confederation, Hillingdon CIC is seeking an experienced General Practice Nurse to join North Connect PCN at Eastbury Surgery. The post holder will work at enhanced practice level, delivering person-centred care across long-term conditions and complex health needs.

This part-time role (25 hours per week, permanent) offers autonomous working within a supportive team and opportunities to lead quality improvement and staff development in a London primary care setting.

Responsibilities

Job description

We are looking for an experienced General Practice Nurse to join our North Connect PCN in Northwood., The post holder will be an experienced registered nurse working at enhanced practice level across the Primary Care Network. Working autonomously within an agreed scope of practice, the post holder will provide high-quality, person-centred nursing care for people with long-term conditions, multimorbidity, frailty and other complex health needs. They will use specialist knowledge, enhanced assessment skills and evidence-based clinical decision-making to assess need, develop and review personalised care plans, support self-management and make timely referrals or escalation where required. The post holder will contribute to population health management, prevention, early identification of disease, delivery of PCN priorities and reduction of health inequalities. They will provide clinical leadership within the general practice nursing workforce, support the development of colleagues and lead or contribute to quality improvement, audit and service development. The post holder will recognise the limits of their competence and seek appropriate advice or refer to an advanced practitioner, GP or other clinician when needed.
