Qualifications

• Masters degree in pharmacy (MPharm) or equivalent.
• CPPE pathway completion or working towards it.
• Evidence of CPD.

Responsibilities

• Provide clinical medicines management expertise, including medication reviews and safe prescribing at care transitions.
• Conduct remote or face-to-face structured medication reviews, especially for polypharmacy, long-term conditions or care home residents.
• Reconcile and manage medicines after hospital discharge, addressing unexplained changes.
• Manage long-term conditions within a multidisciplinary team.
• Handle repeat prescription reauthorisation and acute prescription requests.
• Implement systems for safer prescribing.
• Offer patient-facing support on medication queries and adherence.
• Lead clinical audits and quality improvement, including adherence to local and national prescribing guidelines.
• Review medicines safety for unplanned admissions and high-risk groups.
• Advise on the pharmaceutical aspects of new services with medicinal components.
• Provide medication information to practice staff, healthcare teams and patients.
• Monitor compliance with local and national guidelines, including NICE, and support practice formulary updates.
