Job summary

BWM PCN Limited is seeking a part-time Clinical Pharmacist in the Leeds area to join a multi-disciplinary primary care team. The post focuses on clinical medication reviews, medicines optimisation, and collaboration with community and hospital pharmacy services to improve patient outcomes.

Qualifications

• Minimum of 2 years post-qualification experience.
• Accredited Master of Pharmacy (MPharm) degree and GPhC registration.
• Completion or current pursuit of CPPE pathway qualification.

Responsibilities

• Conduct clinical medication reviews for patients with polypharmacy and comorbidities.
• Provide medicines optimisation guidance to the practice and pharmacy teams.
• Manage repeat prescribing and monitoring to reduce risks and waste.
• Support integration with community and hospital pharmacy services.
