Main duties of the job

Essential Requirements

Registered Pharmacist with the General Pharmaceutical Council GPhC

Must have completed an approved 18-month training pathway (such as CPPE Clinical Pharmacist pathways) that equips you to practice safely in primary care

CPPE Primary Care Pharmacy Pathway completion is mandatory (requirement under Additional Roles Reimbursement Scheme)

Clinical experience in a healthcare setting

Strong communication and interpersonal skills

Ability to work independently and as part of a multidisciplinary team

Commitment to continuing professional development and clinical supervision

Computer literacy and familiarity with clinical systems

Desirable Requirements

Previous experience working in general practice or primary care

Experience with clinical medication reviews and chronic disease management

Knowledge of local prescribing guidelines, formularies, and antimicrobial stewardship

Independent prescribing qualification

Key AccountabilitiesPatient Care and Clinical Assessment

  • Conduct comprehensive clinical assessments of patients with complex medication needs
  • Provide patient-facing consultations using expert knowledge of medicines for specific disease areas
  • Deliver evidence-based pharmaceutical care tailored to individual patient needs
  • Assess medication-related problems and provide appropriate interventions

Prescribing and Medication Management

  • Act as an independent prescriber (if qualified) working with and alongside the general practice team
  • Prescribe medicines within scope of practice and competence (qualified prescribers only)
  • Work collaboratively with GPs and practice team to support medication decisions
  • Provide clinical recommendations for medication therapy
  • Review and advise on medication regimens and therapeutic options

Chronic Disease Management
