Job summary

Pendle East PCN is seeking an experienced Clinical Pharmacist to join our expanding Neighbourhood Team. You will work closely with GPs, practice pharmacists and the PCN team to optimise medicines management and support patients in the community.

You will have a strong background in GP practice, with Medicines Optimisation, and be prepared to develop into a prescriber role as part of our ongoing service expansion.

Qualifications

• Registered Pharmacist with GPhC and 5+ years post-registration experience.
• Experience working in GP practice and medicines management.
• Commitment to continuous professional development and audit.

Responsibilities

• Clinically assess and treat patients using medicines knowledge for disease areas.
• Undertake Structured Medication Reviews to optimise medications.
• Be a prescriber or train to become one.
• Support transfer of care between hospitals and GPs with appropriate signposting and medication changes.
• Lead medication reconciliation post-discharge with multi-team collaboration.
• Address high-risk drug queries and monitor medications.

Job description

Are you forward thinking, looking for a new opportunity, keen to develop? Then this could be for you! The successful Pharmacist will work closely with our local GPs, existing practice and PCN Pharmacists and prescribing administrative support teams. As our PCN continues to grow and develop this is a really great time to be involved in developing our services for the future. The duties of this post will be subject to regular review, at least on an annual basis. The duties will reflect the requirements from the PCN DES and the GP contract in relation to prescribing and medicines management. These will include but are not limited to:

Duties

• Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
• Undertake Structured Medication Reviews to ensure patients take their medications correctly and get the best from them, reducing waste and promoting self-care.
• Be a prescriber, or commit to completing training to become a prescriber.
• Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, querying discrepancies and recommending appropriate medication changes.
• Take a lead on the reconciliation of medicines following hospital discharge, working with secondary care colleagues, practice colleagues, patients and community pharmacists to ensure patients receive the correct medicines post-discharge. Provide clinical expertise in medicines advice and addressing both the public and social care needs of patient
• Support colleagues and patients with medication queries and drug monitoring especially with high risk drugs.
• Take responsibility for the medicines management of some patients with complex chronic diseases.
• Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
• Work with patients who need support reducing long term prescribed Opiates and Gabapentinoids.
• Help tackle local inequalities and review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit, individual patient reviews and action planning.
• Contribute pharmaceutical advice for the development and implementation of new services that have medicinal aspects (e.g. advice on treatment pathways and patient information leaflets).
• Contribute to the General Practice Quality and Outcomes Framework (QOF) and enhanced services.
• Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.
• Take a lead role in supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
• Take a central role in the clinical aspects of shared care protocols and liaison with specialist, community and anticoagulation pharmacists.

As our Primary Care Network develops in Pendle East, we are looking for an experienced, highly motivated Clinical Pharmacist to join our expanding Neighbourhood Team.

You will have a minimum of 5 years post-registration experience, and will use your expert knowledge of medicines to support our Primary Care Network in providing effective patient services to residents of Pendle East. You will also contribute to the continued learning of PCN colleagues.

• Undergraduate degree in Pharmacy
• Registration with the GPhC, Minimum 5 years post registration experience as a Pharmacist, More than 5 years post registration experience as a Pharmacist
