This is a fixed-term 12-month contract for a Clinical Pharmacist to work within our established PCN pharmacy team across the Network Area. The role is based between the neighbourhoods, responding to the needs of member practices. The post holder will be a pharmacist acting within their professional boundaries, providing clinical support within the PCN and working as part of multidisciplinary teams in general practice. They will receive appropriate clinical supervision. Responsibilities include ensuring practice integration with community and hospital pharmacy, managing aspects of the quality and outcomes framework, undertaking clinical medication reviews, and representing the PCN on medicines optimisation and quality improvement initiatives. The role also involves supporting general practice staff with prescription and medication queries, with potential development towards non-medical prescribing.

The Post Holder Will:
Qualifications (Essential): Undergraduate degree in pharmacy, registered with the General Pharmaceutical Council.
Experience (Essential): Awareness of common acute and long-term conditions seen in general practice.
Disclosure and Barring Service check and current UK professional registration are mandatory.
