Job description

We have an exciting opportunity for full or part-time, Care Coordinator(s) to join our dynamic and friendly team. The post holder is a Care Coordinator who acts within their professional boundaries to support patients with the knowledge, skills, and confidence to manage their health and wellbeing.

This role requires the individual to work until 6.30pm. Part time and full time applications welcomed.

Main duties of the job

The PCN Care Coordinator will provide extra time, capacity and expertise to support patients in preparing for clinical conversations or in following up discussions with primary care professionals. You will work closely with GPs and other primary care colleagues within the primary care network (PCN) to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers (if appropriate), and ensuring that their changing needs are addressed. Care coordinators will focus on the delivery of personalised care to reflect individual patient needs, local PCN priorities, health inequalities and at-risk groups of patients.

Post holder will holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person.

Post holder will also provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals

Based within South Fulham PCN, you will work closely with GP Practices within the primary care network.

Job responsibilities

The main role of the successful applicant will be to proactively communicate and coordinate information between patients, their carers, doctors and professionals and to assist the clinical teams with administrative functions within the practice.

Person Specification
Qualifications
  • NVQ Level 3, Advanced level or equivalent qualifications or working towards
  • Demonstrable commitment to professional and personal development
  • Experience of working in Primary Care
Experience
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement/ health coaching
  • Experience of supporting people, their families and carers in a related role (including unpaid work)
  • Experience of health and social care assessments
  • Communication and relationship building
  • Demonstrated successful outcomes delivery within determined timeframes
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Ability to listen, empathise and provide person-centred support in a non-judgemental way
  • Able to support people in a way that inspires trust and confidence, motivating others to reach their potential
  • Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies and stakeholders
