Care Co-ordinator Opportunity at Mid Norfolk Primary Care Network (PCN)

Mid Norfolk PCN has an opportunity for a care co-ordinator to join their team, supporting six GP practices and linked care homes across the Dereham area. This position can be considered on a full or part-time basis.

The role involves working closely with GPs and practice teams within the PCN, ensuring all patients receive the best possible care and service.

Main Duties:
  1. Act as a central point of contact to ensure appropriate support is available.
  2. Plan and organise work using own initiative, while being able to work as a valuable team member.
  3. Make referrals to relevant service providers.
  4. Coordinate meetings, prepare actions beforehand, and ensure relevant staff are present.
  5. Promote and facilitate integration within GP and community teams.
  6. Handle sensitive information about complex matters with confidentiality.
  7. Create and distribute detailed minutes from meetings, highlighting actions for individuals.
  8. Provide support and guidance to patients and families/carers when required.
  9. Maintain accurate electronic records of patient care, including hospital admissions and discharges.
  10. Signpost service users and carers to relevant services.
  11. Contribute to quality audits.

Skills, Experience, and Qualifications:

Essential:

  • Excellent knowledge of Microsoft Office.

  • Experience with office procedures and database management.
  • Experience working within multidisciplinary teams.
  • Knowledge of relevant health and social care legislation.
