Join a primary care network in Bury St Edmunds as a care coordinator, supporting vulnerable groups and collaborating with GPs. This full-time role involves developing care plans, managing patient interactions, and ensuring optimal care delivery across health services.
Bury St Edmunds PCN has an opportunity for a care coordinator to join our primary care network (PCN), supporting five GP practices across the Bury area. The role is full-time, 37.5 hours per week, mainly Monday and Friday, between 08:30 and 18:00, with some weekend hours as needed.
The role involves working closely with practices, the multidisciplinary team (MDT), and the wider PCN to ensure patients receive optimal care, supporting the PCN Direct Enhanced Service Contract (DES).
Our care coordinators proactively identify and support vulnerable groups, including the frail/elderly and those with long-term conditions, coordinating care across health and social services.

Bury St Edmunds Primary Care Network includes five surgeries serving approximately 67,000 patients from the town and surrounding villages.
