Job description

Bury St Edmunds PCN has an opportunity for a care coordinator to join our primary care network (PCN), supporting five GP practices across the Bury area. The role is full-time, 37.5 hours per week, mainly Monday and Friday, between 08:30 and 18:00, with some weekend hours as needed.

The role involves working closely with practices, the multidisciplinary team (MDT), and the wider PCN to ensure patients receive optimal care, supporting the PCN Direct Enhanced Service Contract (DES).

Our care coordinators proactively identify and support vulnerable groups, including the frail/elderly and those with long-term conditions, coordinating care across health and social services.

Main duties of the job
  1. Work with patients, families, and carers to enhance understanding of health conditions and develop personalized care plans.
  2. Assist patients with queries, appointments, and provide quality information to support decision-making.
  3. Collaborate with GPs and primary care staff to manage a caseload, including patients with long-term conditions.
  4. Support multidisciplinary team coordination within the PCN.
  5. Promote shared decision-making and prepare staff and patients for these conversations.
  6. Develop communication channels among GPs, patients, families, carers, and other agencies.
  7. Maintain records of referrals and interventions for monitoring and evaluation.
  8. Help keep care records current by updating patient information.
  9. Contribute to risk assessments, service monitoring, and evaluation.
  10. Work with commissioners and local agencies to develop the role further.
