CORNWALL PARTNERSHIP NHS FOUNDATION TRUST is seeking a clinical professional to support the patient journey in inpatient mental health services. The role focuses on safe and timely discharge planning and transfer of care, working within a multidisciplinary team and managing a caseload delegated by supervision.

The post holder will play a pivotal role in supporting the patient journey through mental health inpatients services. The aim is to provide a collaborative approach to hospital discharge and is to ensure that patients only stay in hospital for the time required to complete their treatment, and they experience timely and seamless transfer of care on discharge., Supporting professionals, carers and patients to develop safe and sustainable discharge plans. The post holder will have responsibility for actively participating as a key member of the multi-disciplinary team. The post holder will manage a case load as delegated by supervisor.
The post holder will work closely with the bed coordinators to support the flow through the mental health wards.
