Lovemead Group Practice in Bath is seeking a Respiratory Nurse to join the clinical team. You will review and manage patients with asthma and COPD, conduct annual reviews, and perform spirometry where qualified. You will work with GPs, pharmacists and a multidisciplinary team to deliver personalised care plans.

Qualifications

• Current NMC registration as a Registered Nurse.
• Experience managing patients with asthma and/or COPD.
• Experience in primary care, community nursing, or general practice.
• Experience delivering patient-centered care and health promotion.

Responsabilités

• Manage and review patients with asthma, COPD and other respiratory conditions, including respiratory and annual reviews.
• Assess, diagnose and manage patients within own scope of practice.
• Perform and interpret spirometry where qualified and competent.
• Develop, implement and review individualised care and self-management plans.
• Support early identification, diagnosis and management of respiratory disease, escalating and referring where needed.
• Deliver patient education on respiratory health, inhaler technique, medication adherence and smoking cessation.
• Take part in chronic disease management programmes and multidisciplinary reviews.
• Support achievement of QOF, Enhanced Services and other contractual respiratory requirements.
• Take part in clinical audit, quality improvement and service evaluation.
• Work collaboratively across the multidisciplinary team and with external healthcare professionals, including training colleagues where appropriate.
• Maintain accurate, contemporaneous clinical records.
• Promote equality, diversity and inclusion, and follow practice policies and clinical governance standards.
