Job summary

Hertfordshire Community NHS Trust is seeking a Diabetes Specialist Nurse (Band 7) for 18.75 hours per week. The role covers community diabetes care in South and West Hertfordshire, with clinics and group education sessions. The post includes a 5% High Cost Area supplement plus pro rata £2,122 per annum, with a proposed interview date of 12 November 2026.

Employment is permanent. The successful candidate will join a multidisciplinary team, work across community and acute settings, and enjoy

Qualifications

• Post graduate experience working within Diabetes.
• Experience of providing education.
• Excellent organisation and planning skills.
• Independent prescriber.

Responsibilities

• Deliver diabetes care in community clinics (face to face or telephone).
• Provide group education sessions.
• Work as part of the MDT across community and acute settings.
• Participate in hybrid working to support work/life balance.
