Working with Urgent Crisis Response teams, advanced nurse and paramedic assessors and specialist respiratory and heart-failure services, you will complete telemedicine assessments and support coordinated, responsive care planning.

This varied role includes both bed-based and community acute work, with the chance to develop interests in older people's medicine and integrated care. A 1:4 weekend rota provides cover across the Stamford Unit and Hospital at Home services.
