Job summary

Change Mental Health invites applications for a part-time Community Link Worker in Nairn, Scotland. The role focuses on supporting people affected by mental health issues, coordinating with NHS Highland Primary Care and linking to community resources to improve holistic health outcomes.

The post involves a 21-hour week with flexible days, reporting to a Senior Community Link Worker; SVQ2 or equivalent health and social care experience is preferred.

Qualifications

• SVQ Level 2 or relevant experience in health and social care.

Responsibilities

• Support people to improve their mental and physical health outcomes.
• Embedded with NHS Highland Primary Care services and develop links to community resources.
• Collaborate with GP practices, Practice Nurses and Primary Care teams to link people to community services.
• Manage a caseload using Personal Support Planning to help people reach health goals.
