Job summary

ST HELENS BOROUGH COUNCIL is seeking a motivated Assessment and Review Worker to join the Front Door Service within Contact Cares at Whiston Hospital on a part-time basis. The role is part of a large, multi-disciplinary team that handles health and social care referrals and provides both planned and urgent assessments.

The post requires an Enhanced DBS Disclosure, driving licence, and access to a car. The team works to deliver an 8:00am-10:00pm rota across 7 days, with 18.5 hours per week.

Qualifications

• Enhanced DBS Disclosure required.
• Must be able to drive, have access to a car and hold a valid licence.

Responsibilities

• Provide initial contact, care planning, onward referrals and signposting.
• Deliver a 2-hour response under Urgent Community Response Criteria and conduct planned and urgent assessments.
• Work pattern includes 18.5 hours per week across 7 days.
