Job summary

Dorset Clinical Commissioning Group in Poole is seeking a Parkinson's Specialist Practitioner (Band 6) to provide a weekday service for people with Parkinson's. You will work closely with Neurologists, Older People's Services Consultants, Pharmacists, General Practitioners, Therapists, Social Services and voluntary groups to enhance care and quality of life for people with Parkinson's.

You will manage your own clinics and undertake domiciliary visits with the support of the Parkinson's Service

Qualifications

• Degree-level qualification in a relevant field.
• Mentoring qualification.
• Experience working independently in a Band 6 role.

Responsibilities

• Provide a weekday Parkinson's Specialist service.
• Collaborate with Neurologists, Older People's Services Consultants, Pharmacists, GPs, Therapists, Social Services and voluntary groups to improve care.
• Manage practitioner-led clinics and domiciliary visits with support from senior team.
• Educate patients and families on long-term condition management.
