Job summary

Aylesford Medical Centre in Kent is seeking a Paramedic Practitioner to join a small team delivering on-the-day urgent care. The role focuses on autonomous assessment, diagnosis and treatment, with telephone triage, home visits and care home visits.

You will work within a GP-led team using EMIS Web, with independent prescribing and an interest in women's health welcomed. Registration as a paramedic with the HCPC is required and current UK registration is mandatory.

Responsibilities

• Assess and treat patients autonomously, prioritising health issues.
• Conduct telephone triage and manage referrals or treatment.
• Perform home visits and care home visits as required.
• Identify patients at risk of long-term conditions and refer appropriately.
• Provide care in line with NICE, NSF and evidence-based practice.
• Act as lead clinician in clinical emergencies and coordinate follow-ups.
• Support medicines use and liaise with external services for vulnerable patients.
• Maintain accurate clinical records using Read codes.

Job description

Paramedic practitioner post at Aylesford Medical Centre, a GMS practice of about 7,500 patients, providing on-the-day urgent care. The work centres on autonomous assessment, diagnosis and treatment, with telephone triage, surgery appointments, home visits and care home visits. The role sits within a team of GPs, nurses and HCAs using EMIS Web and the ABC PCN, with independent prescribing and an interest in women's health both welcomed. Registration as a paramedic with the HCPC is required.

What the job involves

• Assessing, diagnosing and treating patients autonomously, prioritising health issues
• Telephone triage and treating or referring patients who attend or call
• Triage and treatment during home visits and care home visits
• Identifying patients at risk of long-term conditions and signposting or referring them
• Routine care in line with NICE, NSF and evidence, reviewing and changing treatment as needed
• Acting as lead clinician in clinical emergencies
• Ensuring continuity of care, arranging follow-ups and reviews
• Recognising, assessing and referring patients with mental health needs
• Supporting medicines use, liaising with external services for vulnerable patients
• Supporting safeguarding, following PGDs and local pathways, and referring substance misuse
• Collecting pathology specimens and chaperoning patients
• Keeping accurate clinical records, using Read codes and delivering health promotion
